Adult Day Services (Day Habilitation) provides a wide range of health, social, and other
related support services at two locations: Westwood
and West Roxbury. The Adult Day Services at SNCARC have been ACCREDITED by CARF, the Commission on Accreditation of Rehabilitation Services.
SNCARC provides Adult Day Services (Day Habilitation) that are funded
through Medicaid and are designed to provide therapies and training in
functional living skills. An individually designed support plan (ISP) with specific
goals and objectives is developed, with participant input, for each person in the
program. Individuals seeking vocational services are
referred to programs such as SNCARC's sister agency, LIFEWORKS.
SNCARC’s Day Habilitation Program has a range of options from which to
choose for individuals looking for services to match their interests and
needs. In addition to the main part of the program, with its emphasis on functional skills, community
outings, volunteer opportunities, physical, occupational and speech therapies, along with nursing, there are two other related SNCARC components
for specific groups: the ALTERNATIVE DAY PROGRAM and
the SENIOR CONNECTIONS PROGRAM.
Support for Caregivers
Caring for a person who is frail or has a disability is a demanding job. Many caregivers are responsible for providing adult care and raising children at the same time. Adult Day Services provides respite for caregivers, while their family members enjoy a measure of independence in a supportive environment during the day.
